I understand that massage therapy is for relaxation and wellness only and is not a substitute for medical treatment or diagnosis.
I have informed the therapist of all relevant health conditions and will update them of any changes before future sessions.
I agree to communicate any discomfort, pressure preferences, or concerns during the session.
I understand that massage is entirely non-sexual in nature, and inappropriate behavior of any kind will result in the immediate termination of the session, with full payment due.
I will provide at least 24 hours’ notice to cancel or reschedule appointments. Late cancellations or no-shows may be charged the full session rate.
I release the massage therapist from any liability for injury or condition that may arise from the massage, except in cases of gross negligence.
I consent to receive massage therapy and assume full responsibility for my own health during and after the session.
